To report a breach of security or an exam irregularity, you may do one of the following. You are not required to provide your name if you wish to remain anonymous.

•	Call 800-250-3196, extension 5296, and leave a message. Please provide enough details to allow NCEES to fully investigate the incident; or
•	Call 800-250-3196, extension 5467, and ask to speak to the Compliance and Security Manager; or
•	Complete the form below.
