The requirement to keep adequate records is the responsibility of the licensee and must be maintained by the licensee for a period of three years. Records required include, but are not limited to: (1) a log showing the type of activity claimed, sponsoring organization, location, duration, instructor's or speaker's name and PDH credits earned; and (2) attendance verification records in the form of completion certificates or other documents supporting evidence of attendance. PDH records for each license hold must be maintained for a period of three years by the license holder. A license holder must certify that they have met the PDH requirements for renewal. The license holder does not need to send in copies of all documentation for renewal. Copies may be requested by the board for audit verification purposes. |
